I am a real person and I go to the local target. Or perhaps I attend a convention that is happening. Point being, I am highly recognizable and there is a chance you might see me in public (it has happened numerous times).​HIPPA prevents me from initiating contact first. It’s up to you what happens, I am never offended if you want to pretend like we have never met instead of saying hi!​I see a lot of friends and families for different patients so I like to remind everyone that my lips are SEALED! I take pride in approaching things in a non-biased way. I will never talk about you to someone else you may know if they are seeing me as well.

Medication Refill

You can either have your pharmacy send me a refill request or head back to the main page and click the REFILL REQUEST button. ​If you do click the REFILL REQUEST button on the main page, please make sure you give enough info if you are switching pharmacies so I make sure to send your prescription to the right place! If it is the same pharmacy as usual, just put the name (Walgreens, CVS, etc) and the street name. I have your usual pharmacy on file already. ​If we start a new medication that you have never been on before, I require that I see you at least once before sending a refill. It is my job to catch anything negative!​Need a refill but can’t make your appointment? Life happens! As long as nothing major has changed, and I have seen you for at least one follows up visit on a medication, please always request a refill. I never want you to cold turkey your medication! My only requirement is that I see individuals at least once every six months to continue sending refills (control substances require a visit every three months though).

Questions Between Visits

If you have multiple questions that require a back and forth conversation, please schedule an appointment with me. Sometimes things are difficult to explain via messaging and with how busy I am, it might take days to have a conversation because I can only respond between visits. ​If you have one or two questions that I can respond to easily (like yes or no questions), you can always reach out. It is an open door policy!​Currently, you can message Terri or Nicole on the SimplePractice portal. They forward the message to me and I respond within the day if possible. If it is regards to billing or scheduling they will handle it!​You are always welcome to call the main number, 630-839-9199, as well and someone can help direct your question as needed.

Need Paperwork Completed?

If you need paperwork completed, such as a note for school about your diagnosis or a travel note that includes a list of your medication, that can be completed during the time of your visit. If they can’t be completed during your visit, I will let you know if we need a follow up or if I am able to complete it and send it to you. An ESA letter doesn’t take much time obviously. It just all depends on what the situation is. If something pops up and you need something outside of a visit, head over to the link for a "form request" and let me know what you need. I will try to accommodate!​If you need more extensive paperwork completed, such as FMLA, this might require a follow up appointment to complete. I like to do these together because I want to make sure that all the information and dates are correct!​All paperwork that is completed will be uploaded to your chart. You will get an alert that says something like, “Your provider sent a new document for you to review”. Please review the document promptly and alert me to any changes that need to be made. Appointments are not needed for small changes.

Appointments, Frequency, and No Shows

Follow Ups are just a formality and questioned based on when I think we can make a decision on something. Different medications have different timelines. At first, these might be every 2-4 weeks as we make changes. As time goes on and we get closer to symptom control we will start to push them out further.​You can always see me before your next appointment if needed! The SimplePractice app allows you to see my availability and request and appointment. Typically I am in the office Mondays and Fridays but I do telehealth every day! Parents: If you need to see me while your child is at school (or during after school activities) that is ok! However, if I do not see them at that appointment, I do like that I will see them the next time. Just a heads up!​I always set aside 30 minutes for follow ups. If we end early, that is totally fine! Visits that go over 15 minutes, even if it is just by 1-2 minutes, are still billed at the 30 minute increment.​No-Show visits are marked after 15 minutes. The first time is waived, it happens to the best of us! After that, Fulfill Health policy states that a no-show fee will be charged.
